The parameters of this book did not permit me to include as many references as I would have liked in the print edition. Here you find all of the notes included in the print edition plus supplementary information, references, and links to primary sources. I am hopeful that this will make it easier for readers to find the information that they are looking for. I am grateful to Little, Brown and Company for allowing me to include all of the notes here.
How to Use:
Go to the notes of the section you are reading (e.g. Part One, Chapter One). Scroll down to the page number (and, if applicable, note number) of the section that you are interested in. Each individual note is identified by page number, note number, and a brief quotation from the book.
How to Cite:

In the above p. = page number and n.= note number. The note number will either be the note as printed in the book and expanded here (e.g. n.10, n.11, n.12) or an additional note exclusively located here on the website (e.g. 10a, 10b, 10c).
